MultiCare Connected Care (MCC), one of the leading clinically integrated care networks in the state of Washington, to improve the health of populations in the Pacific Northwest has announced a new partnership with healthcare technology company Innovaccer. The collaboration will focus on implementing a care management framework for MCC’s Medicare Shared Savings Program (MSSP) Track 1+ patient population. MultiCare Health System is a minority investor in Physicians of Southwest Washington (PSW). MCC and PSW have partnered to improve healthcare delivery and management and lead the way to healthier futures.
FHIR-enabled Data Activation Platform
With Innovaccer’s FHIR-enabled Data Activation Platform, MCC plans to enhance its existing operational capabilities and capacities for population health. MCC will be deploying the FHIR-enabled Data Activation Platform to unify the medical records of more than 37,000 Medicare patients, gathered from multiple data sources and will utilize them to manage beneficial health initiatives to ensure the wellbeing of the population.
In addition, Innovaccer’s population health analytics solution, InGraph, will enable MultiCare Connected Care to monitor utilization across the network and reduce costs for its employed and independent providers. MCC will also be implementing InCare, Innovaccer’s Artificial Intelligence-assisted care management solution, to ensure real-time care coordination among clinicians to equip them with efficient automated workflows.

MultiCare Connected Care (MCC) is an independent Accountable Care Organization (ACO) established in 2014 as a wholly-owned subsidiary of MultiCare Health System and operates as an independent entity. MCC has established a comprehensive Clinically Integrated Network (CIN) comprised of doctors and other health care providers, as well as hospitals, clinics, and other health care services, such as imaging, labs, and pharmacies.
