– The report casts doubt on the effectiveness of widely used digital diabetes management tools. Despite billions in investment, the study finds these tools offer minimal health benefits and actually increase healthcare spending.

Millions Rely on Tools, But Evidence Shows Little Impact

The PHTI analysis focused on eight popular tools for tracking blood sugar (non-continuous glucose monitors) used by people with Type 2 diabetes. While these tools promise better health outcomes and cost savings, the research paints a different picture:

  • Small, Unsustained Improvements: HbA1c reductions, a key diabetes measure, were minimal (0.23 to 0.60%) and didn’t last over time. These results fall short of the industry standard for meaningful change.
  • No Broader Health Benefits: The study found no significant improvements in weight, blood pressure, cholesterol, or other diabetes-related health factors.
  • Increased Costs for All Payers: The price of these tools outweighs any cost savings because the minimal benefits don’t prevent other treatments or care. Annual spending is projected to increase across all insurance types:
    • Commercial: +$2,002
    • Medicare: +$1,011
    • Medicaid: +$723

Bright Spots and Recommendations

The report identifies two potential areas for improvement:

  • Virta’s Nutritional Ketosis Approach: Early data suggests Virta’s program, which requires strict dietary changes, may lead to diabetes remission for some patients.
  • Early Intervention with High-Risk Patients: Digital tools might be more impactful for newly diagnosed patients with high starting HbA1c levels, helping them establish better self-management habits.

PHTI offers recommendations for the future of digital diabetes management:

  • Focus on Clinically Meaningful Benefits: New tools should aim for substantial HbA1c improvements, potentially integrating continuous glucose monitors and new medications.

The program, launched in partnership with CVS Caremark and Livongo, is projected to significantly improve health care outcomes for thousands of people with diabetes covered by the state plan by providing them with comprehensive around-the-clock live coaching and support through cellular-enabled diabetes management devices with remote monitoring capabilities.


The State of Connecticut Comptroller Kevin Lembo today announced the launch of a diabetes management program powered by Livongo through the state health plan aimed at reducing acute emergencies and long-term complications related to chronic conditions while delivering an estimated annual savings to the state of approximately $3 million. More than 500 people have already signed up for the program during a two-week soft launch period, and more are enrolling each week.

CVS/Livongo Partnership

The program, launched in partnership with CVS Caremark and Livongo, is projected to significantly improve health care outcomes for thousands of people with diabetes covered by the state plan by providing them with comprehensive around-the-clock live coaching and support through cellular-enabled diabetes management devices with remote monitoring capabilities.

As state comptroller, Lembo is the administrator of the state health plan, which serves more than 250,000 state and municipal employees, retirees and their dependents. This diabetes management program is the latest cross-sector partnership that seeks to enhance the efficiency of the state plan by helping members to better manage chronic conditions.

Livongo Diabetes Management Program Overview

The Livongo diabetes management program and devices will allow members to monitor their conditions, receive Health Nudges™, and 24/7 access to digital and telehealth coaching, all from the safety of their homes. The program is modeled after similar efforts that have a proven track record of success. A Journal of Medical Economics study found that the Livongo for Diabetes program delivered an $88 per member monthly reduction in medical spending and that clients experienced a positive return on investment in one year.

Benefits for Plan Members

The program is designed to provide several benefits to plan members and the state by:

– Providing more supportive, successful preventative care to members at no cost to them that results in better health care outcomes;

– Addressing the state’s top pharmacy cost category (diabetes medication and supplies);

The U.S. Food and Drug Administration (FDA) has permitted marketing of the Tandem Diabetes Care t:Slim X2 insulin pump with interoperable technology (interoperable t:Slim X2) for delivering insulin under the skin for children and adults with diabetes. This new type of insulin pump, Referred to as an alternate controller enabled (ACE) infusion pump, or ACE insulin pump, is the first interoperable insulin pump, meaning it can be used with different components that make up diabetes therapy systems, allowing patients to tailor their diabetes management to their individual device preferences.

The FDA reviewed interoperable t:Slim X2 pump performance data demonstrating that the device can dose insulin accurately and reliably and at the rates and volumes programmed by the user. The FDA also assessed the ability of the pump to communicate with external devices with appropriate reliability, cybersecurity and fail-safe modes.

Diabetes therapy systems may be comprised of an ACE insulin pump and other compatible medical devices, including automated insulin dosing (AID) systems, continuous glucose monitors (CGMs), blood glucose meters or other electronic devices used for diabetes management.

t:Slim X2 Insulin Pump Overview

The interoperable t:Slim X2 pump works by delivering insulin under the skin at set or variable rates. It can be digitally connected to automatically communicate with and receive drug dosing commands from other diabetes management devices, such as AID systems, or, when not connected to other devices, the interoperable t:Slim X2 pump can be used to infuse insulin on its own. AID systems typically consist of a pump, CGM, and software to control the system.

Insulin pumps to date have either been cleared by the FDA as stand-alone devices (class II, moderate risk devices) or approved by the FDA as part of a single, predefined diabetes management system (class III, highest-risk devices). Because the interoperable t:Slim X2 insulin pump is interoperable with other diabetes device components, the pump was reviewed through the de novo premarket review pathway, a regulatory pathway for novel, low-to-moderate-risk devices of a new type.

Along with this authorization, the FDA is establishing criteria, called special controls, which outline requirements for assuring the accuracy, reliability, cybersecurity and clinical relevance of ACE infusion pumps, as well as describe the type of studies and data required to demonstrate acceptable pump performance. These special controls, when met along with general controls, provide reasonable assurance of safety and effectiveness for devices of this type. With the authorization of the interoperable t:Slim X2 insulin pump and the establishment of these special controls, the FDA’s action also created a new regulatory classification, which provides more efficient patient access for this type of device in the future, because future ACE insulin pumps that comply with the general and special controls can go through a more efficient premarket review known as 510(k) clearance.

“Diabetes is a complicated disease that requires close monitoring and carefully tailored treatments. We’ve heard from the patient community that having the ability to customize their own diabetes management devices is important to them. Advances in digital health make more tailored approaches to diabetes care possible,” said FDA Commissioner Scott Gottlieb, M.D. “The marketing authorization of the first ACE insulin pump intended for interoperable use has the potential to aid patients who seek more individualized diabetes therapy systems and opens the door for developers of future connected diabetes devices to get other safe and effective products to patients more efficiently. Because the FDA’s action creates a new regulatory classification, future ACE insulin pumps will be able to go through the more efficient 510(k) review process, helping to advance this innovative technology.
