As part of the partnership, Buckeye will connect Cityblock providers to members who haven’t seen their primary care physician in a year, promoting preventive care and chronic disease management. Cityblock’s 24/7 wrap-around services provide virtual, in-home, and clinic-based care, extending access and convenience.
